Acne is no longer the exclusive domain of adolescence. Increasingly, dermatology and aesthetic medicine practices are treating patients who are over 25—and even 40—with active breakouts. This condition is known as adult acne, a state that, rather than being a one-off issue, tends to become chronic and disproportionately affects women.
“This is acne that is more persistent, more inflammatory, and located in a very characteristic pattern, especially around the jawline and neck,” explains Dr. Rita Sêco, a specialist in aesthetic medicine. Its appearance is closely tied to hormonal changes that accompany phases such as the menstrual cycle, pregnancy, postpartum, or perimenopause.
Yet reducing it solely to a hormonal matter would be an oversimplification. Adult acne results from a set of interacting factors. “Hormonal dysregulation increases sebum production and promotes the growth of the bacterium Cutibacterium acnes, but genetics, stress, a diet high in sugars or dairy, and the use of unsuitable cosmetics also play a role,” notes the specialist.
There is also the false belief that only oily skin gets it. “Oily skin is naturally more prone because it produces more sebum and has pores that are more dilated. However, adult acne also appears on dry and sensitive skin—often as a response to a damaged skin barrier or hormonal imbalances that do not directly relate to skin type,” notes Sêco.
Prevention, although it may not always prevent its appearance, can indeed make a difference in its progression. Avoiding comedogenic products, managing stress, ensuring proper rest, and maintaining a balanced skincare routine—without excesses or aggressive practices—are fundamental pillars.
Regarding actives, evidence supports the use of ingredients such as adapalene, benzoyl peroxide, salicylic acid, niacinamide, or azelaic acid. Added to these is an non-negotiable step: daily sunscreen, essential not only to prevent acne from worsening but also to avoid post-inflammatory hyperpigmentation. “Adult acne is more persistent and inflammatory and requires a completely different approach from adolescent acne,” affirms the specialist.
Because nothing concerns us as much as the outbreak itself as its sequelae. In the most inflammatory cases, especially when lesions are manipulated, spots or scars can appear. The former tend to improve with time, but the latter require a more targeted approach. “In consultations we have medical treatments such as laser or microneedling, particularly effective for scars, which we combine with at-home routines that include actives like vitamin C, niacinamide, or retinoids,” explains the doctor.
The message, however, is clear: there are no universal solutions. Adult acne demands consistency, professional diagnosis, and a personalized treatment plan. Only then is it possible to restore the skin’s balance and, with it, confidence.
